PoE 2 Limb Modification Guide: Every Mod, Flesh Surgeon Access, and Death Rules

Limb Modification gives a character up to four temporary prosthetic bonuses. This guide explains how to reach the Transcension Device, which arm and leg modifiers can appear, what Tier 3 and Tier 4 change, and exactly when the upgrades disappear.

Verification note

Checked July 25, 2026 against PoE2DB room and modifier data. The reroll behavior is a recent community report and may be patched; verify it before committing a valuable Temple run.

How this guide was checked

Room tiers, device uses, modifier ranges, and death behavior come from the linked PoE2DB data pages. A July 2026 Reddit report is cited separately for the cancel-and-reenter reroll behavior. The three visuals are AI-generated editorial concept illustrations, not gameplay screenshots or official game art.

Flesh Surgeon Tier 3 vs Tier 4

Both tiers provide the Transcension Device. Tier 4 improves the room and gives a second use in the same room.

Room Unique effectiveness Device access
Surgeon's Symphony (Tier 3) 30% increased One Limb Modification use
Surgical Glory (Tier 4) 35% increased Two Limb Modification uses

Quick answer: what does Limb Modification do?

PoE 2 Limb Modification replaces one selected arm or leg with a transcendent prosthetic and grants one additional stat. A character can carry modifications on two arms and two legs, so a complete set can provide four bonuses. The effects continue outside Atziri's Temple, but all obtained transcendent limbs are removed when the character dies and resurrects.

The device is not available in every Flesh Surgeon room. You need Surgeon's Symphony at Tier 3 or Surgical Glory at Tier 4. Tier 3 allows one modification; Tier 4 allows two. Because death removes the whole set, prioritize bonuses that solve a real build problem instead of choosing the highest-looking number.

The important wording is resurrection

PoE2DB states that Limb Modifications are lost when you resurrect after dying. Treat every death-and-resurrection event as a full reset and do not plan a permanent build requirement around these bonuses.

How to unlock the Transcension Device

Build the Flesh Surgeon room family inside Atziri's Temple and upgrade it with Synthflesh Lab support until it reaches Surgeon's Symphony. The Tier 1 and Tier 2 rooms improve unique-monster effectiveness but do not list Limb Modification access. The device first appears at Tier 3.

  1. Place and keep a connected Flesh Surgeon room in the Temple.
  2. Use Synthflesh Lab upgrades to advance the room family.
  3. Reach Surgeon's Symphony (Tier 3) for one device use.
  4. Reach Surgical Glory (Tier 4) for two uses in that room.
  5. Keep the room connected to an entrance route before opening the Temple.

A high-tier room that cannot be reached is worth nothing. Before closing the planner, trace a continuous path from the entrance to the Flesh Surgeon room and check whether any planned room placement creates a loop or blocks the upgrade chain. The site's planner is useful for route proof, but the in-game room description remains the final authority for the current patch.

Every current arm and leg modifier

PoE2DB currently lists six leg modifiers and six arm modifiers. The ranges below are game-data ranges, not a promise that every value will appear on each interaction. Match the limb category first: arm options lean toward offense, block, deflection, curse strength, and presence; leg options lean toward movement, evasion, recovery, and stun protection.

Transcendent leg modifier pool
Leg modifier Range Best fit
Increased Evasion Rating 20–30% Evasion builds
Movement Speed while Sprinting 6–10% Mapping and repositioning
Increased Stun Threshold 15–25% Characters vulnerable to stuns
Reduced movement penalty while using skills 5–10% Mobile channeling or attack/cast setups
Mana regeneration while moving 20–30% Movement-heavy mana users
Damage taken recouped as Life 6–10% Sustained recovery
Transcendent arm modifier pool
Arm modifier Range Best fit
Increased Block chance 8–12% Block-based defenses
Increased Attack Speed 6–10% Attack builds
Increased Cast Speed 6–10% Caster builds
Increased Curse Magnitudes 12–16% Curse-focused builds
Increased Deflection Rating 6–10% Deflection defenses
Increased Presence Area of Effect 15–25% Presence or aura coverage

How to choose the best Limb Modification

Start with the build's failure point. A fast mapper that already kills instantly may gain more from recoup, block, or stun threshold than another small damage increase. A bossing caster may value cast speed, but only if mana and survival are already stable. Sprint speed is useful when travel time matters; it does not automatically improve damage during a stationary boss phase.

  • Choose survival first when one death removes all four upgrades.
  • Use attack speed only for attacks and cast speed only for spells that scale with it.
  • Check whether block or deflection has a build-specific cap or prerequisite.
  • Treat Presence area as a coverage tool, not universal damage.
  • Do not abandon a functional Temple route merely to chase a perfect roll.

Simple priority rule

Fix the stat most likely to prevent your next death, then improve clear speed or damage. A slightly lower defensive roll that keeps the character alive can preserve the entire four-limb set longer.

Replacement, reroll, party, and death rules

PoE2DB describes left and right limbs as using the same modifier pool, with offered numeric values varying. A new modification can replace an existing one in that body slot. Plan replacements carefully: the interaction is best used to improve a weak slot, not to gamble away a bonus that already solves an important problem.

A July 2026 community report says players can cancel the selection, leave the table, and interact again to refresh the offered options without consuming an attempt until a modification is chosen. This behavior is not documented as a permanent official rule and could be changed. Test it with the current interface before relying on it, especially on console or after a patch.

Community reports also indicate the device belongs to the player who opened the Temple rather than every party member. Because this detail is not stated on the core game-data page, treat it as a practical warning rather than a guaranteed specification. If party access matters, confirm it before building the whole route.

Are the bonuses permanent? No; resurrection after death removes them Game-data page

When the Flesh Surgeon route is worth building

Build toward the Flesh Surgeon when the route is already close, the character is likely to stay alive, and at least one modifier would materially improve the build. Skip or delay it when connecting the room would cut off Atziri, a stronger reward room, or the Generator coverage required by the rest of the Temple.

Tier 4 is attractive because two uses can fill or repair two slots in one visit, but the extra investment has an opportunity cost. A stable Tier 3 room with one useful upgrade is better than an unreachable Tier 4 plan. Evaluate the room as part of the whole Temple rather than as an isolated reward.

PoE 2 Limb Modification FAQ

Reach a Tier 3 Surgeon's Symphony or Tier 4 Surgical Glory Flesh Surgeon room in Atziri's Temple, keep it connected, and use its Transcension Device.

Up to two arms and two legs, for four active transcendent limbs.

They are removed when the character resurrects after dying. Treat the set as temporary power, not permanent character progression.

Tier 3 lists one Transcension Device use. Tier 4 raises unique-monster effectiveness and allows the device to be used twice.

A July 2026 community report says canceling and re-entering the device refreshes offers before a choice is made. This may change, so verify it in the current patch.

The best option covers the build's weakest point. Defensive and recovery bonuses often have extra value because one death removes the full set.
